HOW CARE BRIDGE WORKS

Family members reach out to us for a free consultation. We understand your concerns, discuss your parents' needs, and answer any questions about our services.

Step 2: IN-HOME ASSESSMENT

A Care Bridge representative visits the senior citizen at home. We assess daily routines, mobility, medical requirements, social needs, and support expectations.

Step 3: CUSTOMIZED CARE PLAN

Based on the assessment, we create a personalized monthly assistance plan.

Step 4: ONBOARDING & PAPERWORK

Simple registration and service agreement. Emergency contacts, medical information, and communication preferences are documented to ensure smooth support from Day One.
